Before your first visit

Before we see you, we'll send you a link to gather some data about yourself, your condition and insurance details.
Your first visit
For your first visit, please bring the following documents with you:

PHOTO ID
-
Insurance Card
-
MRI / CT / X-Ray reports, if any
-
Referral order from referring physician, if you have one

Insurance referral, if required by your insurance provider
Plan to arrive 15 minutes before your start time, to complete patient data, plus allow one hour for your appointment.
Payment
Except for Medicare patients, payment in full is due at the time of service. We accept major credit cards, checks or cash.
Cancellation policy
You may reschedule or cancel up to 24 hours before your appointment time without penalty.
If you do not cancel at least 24 hours before your appointment, a “no-show” fee is payable.
